Requesting an appointment
All appointment requests must go through our online form to be triaged by a clinician. We encourage you to submit your request as early as possible in the day.
If you would like an appointment to see a clinician for a health issue, complete the ‘I have a health problem‘ form. Fill the form with as much relevant information as possible, to enable us to allocate the most appropriate outcome, which may not always be an appointment with a GP.
For admin or routine care queries complete the ‘I have an admin or routine care request‘ form.
Please note: the online form should NOT be used if you believe your request or query to be urgent, in which case you should call the practice and your request can be handled.

Urgent requests MUST be called through – please do NOT submit an online form for urgent medical queries.
Routine appointments cannot be requested over the telephone.
For patients unable to complete online forms or those with additional needs including our elderly patients, our trained staff can assist in completing the form with you. It is for this reason that we request the phone lines remain free. - Walk in

Appointment types
Appointment offered may be by phone or face to face at the surgery. Capacity permitting, we try our best to accommodate for your preferences.
If you need help with your appointment
Please tell us:
- if you need an interpreter
- if there’s a specific doctor, nurse or other health professional you would prefer
- if you have any other access or communication needs

If you need help when we are closed
You can contact the practice’s ‘Out of Hours’ provider, BADGER for urgent medical assistance when the practice is closed. BADGER can be contacted on 0121 766 2145.
You can also use NHS 111 online or call 111. NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Home visits
Whilst we encourage ALL patients to come to the surgery, we do appreciate this is not always possible. If you are registered as housebound and would like to request a home visit, you need to call the surgery on the day before 11AM. Any requests after this time will not get allocated for a visit.
Please note, you may ONLY request a home visit if you are registered housebound with us (usually due to chronic illness). Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls.
You can also be visited at home by a community nurse if you are referred by your GP.
